Application to become a CDSA

Th
e process to become a CD(SA) is an assessment process. In order to register for this process, you will need to complete the online CD(SA) Application Form. In 2017, these applications can only be completed online. This is done to ensure that when Director Certification receives your application, it is complete.


This form needs to be completed in full before it can be submitted. You therefore need to ensure that you are familiar with the 'Guidelines to completing the CD(SA) Application' before initiating completion on our website. Please note that the CD(SA) Application Form must be competed in one session and cannot be saved and returned to later.

Also ensure that you already have digital copies of all the documents that need to be uploaded before initiating the completion of this form. The following documents need to be ready for upload:

  • Full CV
  • Certified copy of ID
  • Certified copy of highest qualification
  • Certified copy of relevant professional certification(s) – optional
  • Proof of payment
  • Form D.1: Letter of Nomination – Proposer
  • Form D.2: Letter of Nomination – Seconder
  • Form E.1: Directorship Experience
  • Form E.2: Organisation of Substance Checklist
  • Form F: Candidate Declaration
  • Form G: Final Checklist
